What is testosterone replacement therapy?

Testosterone replacement therapy, commonly shortened to TRT, is a prescription treatment for men with a confirmed testosterone deficiency where treatment is clinically appropriate. Its purpose is to replace deficient testosterone and address symptoms that are genuinely related to that deficiency.

Symptoms sometimes associated with low testosterone include reduced sex drive, erectile difficulties, persistent tiredness, low mood, reduced muscle mass, increased body fat and poor concentration. These symptoms are not specific to testosterone deficiency and can also have physical, psychological, lifestyle or medicine-related causes.

For that reason, we do not prescribe from a symptom questionnaire alone. Blood results, your medical history, your symptoms and relevant risk factors must be reviewed together before any treatment decision is made.

Who may benefit from a low-testosterone assessment?

The service is intended for adult men who have ongoing symptoms that could be associated with testosterone deficiency and want a structured clinical assessment.

• Sexual symptoms: reduced libido, fewer spontaneous erections or erectile difficulties.
• Physical changes: reduced muscle mass or strength, increased body fat, or reduced exercise capacity.
• General symptoms: persistent fatigue, reduced motivation, poor concentration, low mood or sleep disturbance.
• Previous results: a low or borderline testosterone result that needs proper clinical interpretation and, where required, repeat testing.

Symptoms alone do not confirm low testosterone. The clinician may identify another likely cause, recommend further investigation or advise you to seek assessment through your GP or another specialist.

How the online TRT service works

   Health review and testing plan

   Your symptoms, medical history, regular medicines, fertility plans and previous results are reviewed. The clinician will confirm which blood panel and any repeat testing are required.

   Comprehensive blood testing

   A prescribing decision requires appropriate laboratory evidence. The PrimeLife pathway may include testosterone, LH, FSH, haematocrit, PSA and relevant metabolic markers, depending on your age, history and clinical circumstances.

   Secure video consultation

   Thomas Harries or Hywel McGlinchy personally assesses your results and clinical history by video. They explain whether testosterone deficiency is supported and discuss suitable next steps.

   Treatment and continuing monitoring

   Where treatment is appropriate, the pharmacist prescriber issues a private prescription and arranges ongoing reviews and repeat blood tests. Treatment may be adjusted or stopped if it is ineffective, poorly tolerated or no longer clinically suitable.

TRT blood-test prices

The clinician will advise which test is appropriate. Package names do not by themselves confirm that every marker needed for your assessment is included, so check the required panel before ordering.

Treatment options and prices

Current treatment prices range from £50 to £240. The exact cost depends on the medicine, formulation and supply agreed after assessment. You will be told the relevant cost before a private prescription is issued.

Choice is clinical, not retail. The lowest price or preferred formulation may not be the safest or most appropriate option for you.

Medicine supply and delivery

Prescription-only medicines are dispensed through Wellness Pharmacy after clinical review and prescriber approval. Booking, paying or completing an assessment does not guarantee that medicine will be supplied.

• UK delivery only: approved orders are sent to UK addresses.
• Tracked delivery: PrimeLife uses Royal Mail Tracked 24 and sends tracking information by email.
• Dispatch: orders are typically prepared and dispatched within one to two business days after prescriber approval.
• Delivery estimate: Tracked 24 often arrives the next day, but this is not a guaranteed next-day service.

Important risks, fertility and monitoring

TRT is not suitable for everyone. Tell the clinician about all medical conditions, regular medicines, supplements, previous hormone use and any current or future fertility plans.

• Fertility: testosterone treatment can suppress sperm production and reduce fertility. Men planning fertility in the near term should discuss this before treatment.
• Blood count: TRT can increase haematocrit or red blood cell levels, which is why blood monitoring is required.
• Prostate monitoring: PSA and prostate-related risk may need assessment before and during treatment, depending on age and individual risk.
• Other possible effects: acne or oily skin, fluid retention, breast tenderness or swelling, mood changes and formulation-specific reactions can occur.
• Conditions requiring particular care: prostate or breast cancer, raised haematocrit, untreated severe sleep apnoea, severe urinary symptoms, uncontrolled heart failure, recent heart attack or stroke, and thrombophilia may make treatment unsuitable or require specialist review.

Seek urgent medical help for severe chest pain, sudden breathlessness, signs of a stroke or another life-threatening reaction. Call 999 in an emergency or use NHS 111 Wales for urgent advice when it is not life-threatening.

What results can TRT provide?

Where testosterone deficiency is properly confirmed and treatment is appropriate, TRT aims to correct the deficiency and improve related symptoms. Some men may notice changes in sexual symptoms, energy, mood, body composition or physical function.

Response varies, improvement is not guaranteed, and symptoms such as fatigue, low mood or erectile dysfunction may persist when they have another cause. Ongoing review is used to assess benefit, side effects and whether treatment should continue.
