Check the cause before treating the symptom
Blocked ears, muffled hearing, ringing, discomfort and hearing-aid whistling can be caused by wax, but similar symptoms can occur with inflammation, infection or other ear problems.
We therefore examine both ears before recommending microsuction. If there is no wax, or removal is not appropriate, the check remains free and we will explain the most suitable next step.

Signs that ear wax may be causing a problem
Ear wax usually clears naturally and does not need to be removed unless it is causing symptoms or preventing a proper examination. A free ear check may be useful if you have:
- Muffled or reduced hearing
- A blocked, full or pressured feeling
- Ringing or buzzing in the ear
- Itching or mild ear discomfort
- A hearing aid that has started whistling

Assessment before removal
Digital otoscopy helps the clinician identify wax, assess the visible ear canal and eardrum, and check for findings that mean treatment should be delayed or avoided.
Removal under direct vision
Microsuction is a dry procedure. The clinician watches the wax and ear canal during removal rather than relying on water to flush the wax out.
Check after treatment
The ear can be examined again after treatment to confirm the result, identify remaining wax and provide clear aftercare or referral advice.
Microsuction compared with ear syringing
Traditional manual ear syringing uses a hand-held syringe to direct water into the ear. NICE advises that manual syringing should not be offered to adults. Treherbert Pharmacy does not use manual ear syringing.
Our TympaHealth microsuction service removes wax without introducing water and is carried out with a magnified view of the ear canal. This can be particularly useful when a dry, visually controlled technique is preferred.
Microsuction is not completely risk-free, and it is not accurate to describe every form of irrigation as unsafe. NICE recognises both microsuction and correctly performed electronic irrigation as options when the practitioner is trained, understands the contraindications and has suitable equipment.

Discuss your symptoms
We ask about your hearing, discomfort, discharge, dizziness, previous ear problems, operations, hearing aids and any treatment already tried.

TympaHealth video otoscopy is used to assess both ears and explain what can be seen. Images can be shown to you where helpful.

Agree the appropriate next step
If wax is present, we explain whether microsuction is suitable, what the procedure involves and the £55 charge. If it is not wax, we provide appropriate advice or signposting.

Microsuction where suitable
A small suction tube is used under direct vision. You may hear loud suction noise or notice tickling, mild discomfort or brief dizziness, and the clinician can pause or stop at any point.

Final check and aftercare
We re-check the ear, explain the result and advise what to do if symptoms persist or return. Most appointments take around 15 to 25 minutes.
Preparing for microsuction
Where suitable, we usually recommend olive oil ear drops for 3 to 5 days before the appointment. Softening hard wax can make removal easier and more comfortable.
Do not use drops if you have a known or suspected perforated eardrum, have been advised not to use them, or they cause pain, discharge or worsening symptoms. Book the free ear check first if you are unsure.
If the wax cannot be removed
Wax can sometimes be too hard, dry, deep or impacted to remove safely at the first visit. We will not force the procedure.
We will explain the next step, which may include further softening, another appointment or referral to an appropriate ear-care service.
When a routine wax appointment is not the right first step
Seek urgent NHS assessment rather than waiting for routine microsuction if hearing loss developed suddenly over 3 days or less, particularly within the last 30 days.
- New facial weakness, drooping or altered sensation on the same side as hearing loss
- Severe ear pain, significant discharge, bleeding or a recent ear injury
- Persistent one-sided or pulsating tinnitus, recurrent vertigo, or rapidly worsening hearing
Use NHS 111 Wales for urgent advice when the problem is not life-threatening, or call 999 in an emergency.

Does microsuction hurt?
Most people tolerate it well. The suction can sound loud and may cause tickling, mild discomfort or brief dizziness. Tell the clinician immediately if you are uncomfortable so the procedure can be paused.
Can both ears be treated for £55?
Yes. The £55 microsuction price covers one or both ears where treatment is needed and clinically suitable.
What happens if there is no wax?
The check remains free. We will explain the visible findings and advise whether pharmacy treatment, an NHS assessment, audiology or another route is appropriate.
Is ear wax removal available through the NHS?
NHS availability varies and not all GP surgeries provide wax removal. Our service is private; we will advise you if the findings need NHS medical review.
