A clinical appointment, not a just administration
Vitamin B12 supports normal red blood cell formation and nerve function. Hydroxocobalamin injections are used to treat or prevent deficiency and may be considered through this private service for eligible adults after assessment.
Tiredness, low energy, poor concentration and pins and needles can occur with B12 deficiency, but they can also have many other causes. An injection cannot be expected to improve symptoms when B12 is not the underlying problem.
We provide a clear suitability check, explain the likely benefits and limitations, and signpost you to GP or NHS assessment when that is the safer route.
Who may be suitable for a private B12 injection?

- General health and wellbeing. A B12 injection can be administered after clinical assessment, but response can vary and not everyone experiences energy, concentration or wellbeing improvement.
- Vegetarian or vegan diet. B12 is naturally found mainly in animal-derived foods, although diet is not always the only cause of deficiency.
- Known B12 deficiency. This includes people diagnosed by their GP, subject to the private service criteria and appropriate NHS signposting.
- Reduced absorption. This may follow gastric or bariatric surgery, Crohn’s disease or a similar condition.
- Medicines that can reduce B12 absorption. Examples include metformin, proton pump inhibitors, H2-receptor antagonists and some anticonvulsants.
- Nitrous oxide use. Nitrous oxide can disrupt B12 function; neurological symptoms require prompt medical assessment.
Symptoms and risk factors for vitamin B12 deficiency
Possible symptoms include unexplained fatigue, difficulty concentrating, memory changes, a sore red tongue, mouth ulcers, pins and needles, numbness, muscle weakness, balance problems and visual changes. Symptoms vary and are not specific to B12 deficiency.
Common reasons B12 may be low
- A diet low in B12-containing or fortified foods
- Autoimmune gastritis or previously diagnosed pernicious anaemia
- Stomach, bowel or bariatric surgery
- Crohn’s disease, coeliac disease or other malabsorption
- Certain medicines or recreational nitrous oxide use
When to seek medical assessment first
- New numbness, weakness, balance difficulty or problems walking
- Visual changes, marked memory or cognitive symptoms
- Severe, persistent or unexplained fatigue or breathlessness
- A known blood result below 150 pmol/L

Injection where appropriate
If suitable, 1 mg hydroxocobalamin is given by intramuscular injection into the upper arm. The injection itself takes only a few moments.

General health and wellbeingEvery 2–3 months
The PGD does not permit a general-wellbeing dose if you have had B12 within the previous two months.
Diagnosed B12 deficiencyClinically directed schedule
Where a GP has diagnosed deficiency, the PGD allows an initial course of up to six doses over two weeks, followed by dosing every 2–3 months. NHS management may be more appropriate.
What are the potential benefits of B12 injections?
For someone with B12 deficiency, replacement can correct the deficiency and may improve symptoms caused by it. Injections bypass absorption through the stomach and bowel, which can be useful when malabsorption is present.
Tablets are suitable for some causes of deficiency, particularly when diet is the main factor. The best route depends on the cause, symptom severity, medical history and clinical assessment. This service does not promise an energy boost, improved concentration, weight loss or any particular response.
When the private service is not suitable
An injection cannot be administered under this PGD if you are under 18, pregnant or breastfeeding, have a known cobalt or cobalamin allergy, Leber’s hereditary optic atrophy, low blood potassium, a cardiac arrhythmia, or are currently undergoing cancer treatment.
Tell us about every medicine and supplement you use, any previous reaction to an injection and the date of your most recent B12 dose. Other exclusions or reasons for referral may be identified during the consultation.
Possible reactions and side effects
- Pain, redness or discomfort at the injection site
- Headache, dizziness, nausea, vomiting, diarrhoea or hot flushes
- Skin reactions or temporary urine discolouration
Seek urgent medical help for an extensive or blistering rash, wheeze, difficulty breathing or feeling faint. Cobalt sensitivity reactions can appear immediately or be delayed for up to 72 hours.

Do I need a blood test before a B12 injection?
Not to book the general private consultation. However, if you are seeking treatment for suspected deficiency, diagnostic blood samples should usually be taken before B12 replacement. We may advise GP testing before giving an injection.
How quickly do B12 injections work?
Response varies according to the cause and severity of deficiency. Some symptoms may improve sooner than others, and an injection may not help symptoms that are unrelated to B12.
Are injections better than B12 tablets?
Not in every situation. Oral B12 may be suitable when diet is the cause or for some medicine-related deficiencies. Injections may be considered when absorption is reduced or a clinician believes the intramuscular route is more appropriate.
Do B12 injections help with weight loss?
No. This B12 service is not a weight-loss treatment and no weight-loss effect is promised.
Can I use the private clinic if I receive NHS B12 injections?
Possibly, but we will review your existing treatment plan and the date of your last dose. You should normally continue the schedule arranged by your GP or specialist unless they advise otherwise.
