WEIGHT-MANAGEMENT TREATMENT GUIDE

How long does Mounjaro take to work?

Some people notice reduced appetite or earlier fullness within the first days or weeks of treatment. For others, the effect is more gradual and becomes clearer after several scheduled doses.

Visible weight loss usually takes longer than the first appetite changes. Mounjaro and Wegovy start at deliberately low doses, so the first month should not be treated as a test of the medicine’s full effect.

Progress should be assessed from the overall trend, treatment use, side effects, appetite, measurements and health—not from one weigh-in.

How long does Mounjaro take to work?

The quick answer

Mounjaro: some people notice appetite changes after the first injection, while others need several weeks or dose steps before the effect becomes obvious. The licensed schedule starts at 2.5 mg weekly for four weeks before moving to 5 mg.

Wegovy: appetite or fullness may change during the first few weeks, but the standard injection schedule gradually rises from 0.25 mg over at least 16 weeks before reaching the established 2.4 mg maintenance stage. Current UK product information also allows a later 7.2 mg option for some adults with obesity when clinically needed; this is not part of every service pathway.

Not feeling a dramatic effect after one injection does not mean that treatment has failed.

What does “working” actually mean?

People often use the word “working” to describe different things. The medicine may be affecting appetite and digestion before a meaningful change appears on the scales.

Appetite changesFeeling satisfied sooner, thinking about food less often or finding large meals less appealing.
Behavioural changesSmaller portions, fewer snacks, improved meal planning or less impulsive eating.
Weight and measurementsA downward trend over several weeks, a smaller waist or clothes fitting differently.
Health improvementsBetter mobility, blood pressure, glucose control, sleep, energy or confidence with food routines.

A strong side effect is not evidence that a medicine is working better. Severe nausea or being unable to eat properly should prompt advice rather than being viewed as a desirable response.

How long does Mounjaro take to work?

Treatment stage What you may notice What matters most
First few days Some people notice earlier fullness or less interest in food. Others feel little difference. Take the correct dose and learn how your body responds. Do not repeat or increase the dose early.
Weeks 1–4 Appetite changes may develop gradually. Weight may fall, remain similar or fluctuate because of fluid and bowel changes. Tolerability, hydration, regular food intake, injection technique and side-effect monitoring.
Weeks 5–8 The next scheduled dose stage may produce a clearer appetite effect, but response remains individual. Review whether side effects are manageable before any further increase.
Weeks 9–12 A more reliable weight trend may now be visible. Some people still require more time before reaching an effective tolerated dose. Assess the whole pattern rather than comparing your result with another person’s dose or weekly loss.

This is a general guide, not a promise of a particular result. Treatment may need to progress more slowly if side effects occur, and some people should not move to a higher dose.

How long does Mounjaro take to work?

Mounjaro contains tirzepatide. It acts on GIP and GLP-1 pathways involved in appetite, food intake and blood glucose regulation. Some people report appetite suppression after the first 2.5 mg dose, but this is not universal.

The 2.5 mg starting dose is used once weekly for four weeks. The product information then moves to 5 mg weekly, with any later increases made in 2.5 mg steps after at least four weeks on the current dose when clinically appropriate.

This gradual schedule means that judging Mounjaro after only one injection—or comparing 2.5 mg with another person taking a much later dose—is usually unhelpful. A patient may notice reduced appetite before meaningful weight loss, and weight loss may continue to build over several months.

Do not use additional injections, shorten the weekly interval or increase the dose without the prescriber’s instruction.

How long does Wegovy take to work?

Wegovy injection contains semaglutide, a GLP-1 receptor agonist. Some people experience earlier fullness or reduced appetite during the first month, but others notice clearer changes only as the dose progresses.

The standard injection schedule begins at 0.25 mg once weekly and usually increases every four weeks: 0.5 mg, 1 mg, 1.7 mg and then the established 2.4 mg maintenance stage from week 17. Current UK product information includes a possible 7.2 mg weekly option for some adults with obesity after at least four weeks on 2.4 mg, but this requires a specific prescribing decision and may not be available through every clinic protocol.

The first 0.25 mg month is therefore not expected to represent the full maintenance effect. A lack of dramatic weight loss in the first few weeks does not automatically mean semaglutide will not help.

Oral Wegovy uses a different daily schedule and administration method. Read our separate guide to oral Wegovy tablets in the UK.

Why might one person respond faster than another?

Individual biology

Appetite regulation, insulin resistance, diabetes, hormones, sleep and genetics can affect response.

Starting point

Initial weight, previous weight loss, eating patterns and weight-related health conditions differ between patients.

Dose and tolerability

Some people remain on a lower dose for longer because of side effects or because that dose is already effective.

Food and drink

Liquid calories, alcohol, grazing, large portions and high-energy foods can reduce the calorie deficit even when appetite feels lower.

Activity and muscle

Everyday movement, resistance exercise and preservation of muscle influence health, function and energy expenditure.

Treatment consistency

Missed doses, incorrect injection technique, poor storage or unregulated products can affect treatment reliability and safety.

How should progress be measured?

Daily scale readings can move because of hydration, salt, carbohydrate intake, constipation, menstruation and exercise. Use consistent conditions and look at a weekly or multi-week trend.

Weight trendWeigh under similar conditions, rather than reacting to a single high or low reading.
Appetite and habitsRecord portion size, snacking, cravings, meal regularity and alcohol intake.
Health markersBlood pressure, blood glucose, mobility and sleep may improve before a major visual change.

When is treatment formally reviewed?

A routine review should happen throughout treatment, not only when something goes wrong. The prescriber will consider weight trend, side effects, adherence, appetite, health changes and whether the current dose remains suitable.

NICE uses longer-term response checkpoints rather than judging treatment after a few weeks. For semaglutide, NICE advises considering stopping if less than 5% of the initial weight has been lost after six months of treatment. For tirzepatide, NICE advises deciding whether to continue if less than 5% has been lost after six months on the highest tolerated dose.

These are NHS guidance points and do not replace an individual private prescribing decision, but they demonstrate why the first week or first month is too early to judge the full response.

Do not try to make the medicine work faster

Taking more than prescribed, injecting early, shortening the weekly interval, using unofficial pen “clicks”, combining GLP-1 medicines or switching products without clinical advice can increase risk.

Severe restriction, prolonged fasting, dehydration and excessive exercise are not safe substitutes for patience and a structured review.

Continue only as prescribed and contact the supplying clinician before any dose or product change.

Frequently asked questions

How long does Mounjaro take to work??

Some people notice appetite changes after the first injection, but others need several weeks or dose stages. The first 2.5 mg dose is a starting dose and does not represent the maximum treatment effect.

How long does Wegovy take to work?

Appetite may change during the first few weeks, but visible weight loss can take longer. Wegovy injection is increased gradually over at least 16 weeks before reaching the usual 2.4 mg maintenance dose.

Should I feel different after the first injection?

Not necessarily. A subtle reduction in appetite is possible, but no immediate sensation does not prove the medicine is ineffective. Continue according to the prescribed schedule.

When should I start losing weight?

Some people record a lower weight in the first month; others show a clearer downward trend over later months. Fluid balance and bowel changes can temporarily hide fat loss.

Does a higher dose always work faster?

No. Higher doses can increase side effects and are not automatically suitable. The right dose is the one prescribed after considering response, tolerability and clinical risk.

Why am I not losing weight on Mounjaro or Wegovy?

You may still be early in dose escalation, the scale may be fluctuating, intake or movement may have changed, doses may have been missed, or you may respond less strongly. Arrange a structured review rather than changing treatment yourself.

Can I transfer treatment from an online provider?

A new provider will usually need to verify the medicine, dose, prescribing history, last administration, current measurements and clinical suitability. Continuation at the same dose cannot be guaranteed.
